Abstract:
A 54-year-old man with autism presented to our hospital for the evaluation of anemia and body weight loss. The patient had mild upper right abdominal pain, and computed tomography revealed wall thickening of the duodenal bulb. Esophagogastroduodenoscopy revealed that a bread bag clip (BC) pierced the pyloric ring and that a duodenal ulcer was present at the puncture site. It was endoscopically removed using grasping forceps. Ingestion of BCs often cause gastrointestinal perforation. We recommend that emergency endoscopy be performed when BC ingestion is suspected.
Insights
A bread-bag clip (BC) caused a duodenal ulcer in a patient with autism. Emergency endoscopy successfully removed the foreign body, highlighting its importance in suspected BC ingestion cases.
Area of Science:
- Gastroenterology
- Endoscopy
- Foreign Body Ingestion
Background:
- Autism spectrum disorder can be associated with pica, leading to the ingestion of non-food items.
- Foreign body ingestion in the gastrointestinal tract can cause significant complications, including perforation and obstruction.
Purpose of the Study:
- To report a case of duodenal ulceration caused by a bread-bag clip (BC) in an adult patient with autism.
- To emphasize the diagnostic and therapeutic role of esophagogastroduodenoscopy in managing such cases.
Main Methods:
- A 54-year-old male patient with autism presented with anemia and weight loss.
- Computed tomography revealed duodenal wall thickening.
- Esophagogastroduodenoscopy identified a BC piercing the pyloric ring, causing a duodenal ulcer.
Main Results:
- The bread-bag clip was successfully removed endoscopically using grasping forceps.
- The patient's symptoms were attributed to the ingested foreign body.
Conclusions:
- Ingestion of bread-bag clips can lead to serious gastrointestinal complications such as duodenal ulcers.
- Emergency endoscopy is a crucial intervention for suspected bread-bag clip ingestion.
