A modified balloon-assisted thrombin injection (BATI) technique effectively treats complex femoral artery pseudoaneurysms. This minimally invasive approach offers a promising alternative for challenging cases.

Background:

  • Iatrogenic femoral artery pseudoaneurysms are common after catheterization.
  • Ultrasound-guided thrombin injection is standard but challenging for wide-neck or complex pseudoaneurysms.

Observation:

  • A modified balloon-assisted thrombin injection (BATI) technique was used in 6 patients.
  • Distal access (retrograde pedal or anterograde radial) allowed simultaneous angiography and balloon occlusion.
  • The technique was applied to pseudoaneurysms with wide necks or high procedural risk.

Findings:

  • Complete thrombosis was achieved in all 6 patients.
  • No complications or recurrence were observed during 6-9 months of follow-up.
  • The modified BATI technique proved effective for complex pseudoaneurysms.

Implications:

  • Modified BATI via distal access is a viable minimally invasive option for complex pseudoaneurysms.
  • This technique may be advantageous when traditional methods are less suitable.
  • Further research is warranted to assess long-term outcomes.
