Summary

Complete mesocolic excision (CME) improves colon cancer outcomes by enhancing surgical precision and increasing lymph node retrieval. This radical approach offers better local control and survival rates for patients with colon cancer.

Background:

Complete mesocolic excision (CME) is a proposed radical operation for colon cancer.
Evidence suggests a survival benefit from CME, but mechanisms are unclear.

Purpose of the Study:

To review literature and examine potential mechanisms behind the oncologic benefits of CME.

Main Methods:

Comprehensive literature review of PubMed and Embase databases.
Examination of potential mechanisms for CME's oncologic benefit.

Main Results:

CME improves radial margin negativity, lymph node yield, and staging.
En bloc removal of mesentery addresses tumor deposits, reducing recurrence and improving survival.
CME is linked to decreased local recurrence and enhanced disease-free/overall survival.

Conclusions:

CME significantly benefits local disease control and cancer-specific survival.
Further research needed on prognostic pathologic and molecular factors in CME.
