Summary

For non-lentigo maligna/non-acral lentiginous melanoma in situ (MIS), clear diagnostic excisional biopsy margins may suffice, potentially reducing overtreatment. Further research is needed to confirm these findings for melanoma in situ recurrence.

Background:

  • Overdiagnosis of melanoma in situ (MIS) is a recognized issue.
  • Limited data exists on the local recurrence rates for non-lentigo maligna (non-LM) and non-acral lentiginous melanoma (non-ALM) subtypes.

Purpose of the Study:

  • To evaluate local recurrence and prognosis for non-LM/non-ALM MIS.
  • To assess the impact of histopathological margin clearance from excisional biopsy.
  • To determine the association between wide excision margin size and recurrence.

Main Methods:

  • Retrospective cohort study of 401 patients with non-LM/non-ALM MIS diagnosed between 1991-2023.
  • Minimum 1-year follow-up at a specialized skin cancer unit.
  • Data collected on demographics, margin status, and treatment, including wide excision details.

Main Results:

  • Only one local recurrence occurred in a patient not undergoing wide excision despite involved margins.
  • 30 lesions with clear excisional margins and no wide excision showed no recurrence after a median of 8.1 years.
  • 23 lesions treated with wide excision margins narrower than standard (0.36 cm) also showed no recurrence.

Conclusions:

  • Diagnostic excisional biopsies with clear margins might be adequate for treating certain melanoma in situ subtypes.
  • The findings suggest a potential to avoid unnecessary wide excisions in select cases.
  • Larger studies are recommended to validate these observations and refine treatment guidelines.
