Primary splenic angiosarcoma is a rare, aggressive tumor, characterized by rapid metastasis and poor prognosis. Herein, we report a case of 18 F-FDG and 68 Ga-FAPI-04 PET/CT imaging in a 57-year-old man with primary splenic angiosarcoma. 68 Ga-FAPI-04 PET/CT presented much higher uptake in the lesions compared with 18 F-FDG PET/CT, showing better diagnostic performance. Our case demonstrated the characteristics of 68 Ga-FAPI-04 PET/CT in primary splenic angiosarcoma and suggested its potential as a more promising alternative to 18 F-FDG PET/CT for diagnosis and a useful tool for clinical management.
