Cystic Fibrosis: Management

220
Cystic fibrosis (CF) is an autosomal recessive disorder that predominantly affects individuals of Northern European descent, occurring at a rate of 1 in 3500. It is caused by a genetic mutation in a gene on chromosome 7, most commonly the ΔF508 mutation, that codes for the cystic fibrosis transmembrane conductance regulator (CFTR) protein. This results in thicker mucus secretions and obstruction pathologies in multiple organs, including the lungs and sinuses.

Area of Science:

  • Pulmonary Medicine
  • Exercise Physiology
  • Clinical Research

Background:

  • Exercise and body composition assessments are vital for managing cystic fibrosis (CF).
  • Current CF care guidelines recommend exercise testing and counseling.
  • Underutilization of these assessments in CF care centers necessitates further investigation.

Purpose of the Study:

  • To develop and validate a comprehensive survey instrument.
  • To explore barriers and facilitators to exercise and body composition testing in CF care.
  • To inform strategies for improving assessment utilization in CF care.

Main Methods:

  • A Delphi methodology was employed with 79 invited experts in exercise and CF.
  • Expert feedback was gathered over three rounds to refine a survey instrument.
  • A final 114-item survey was validated based on expert consensus.

Main Results:

  • Thirteen subject matter experts participated, with six completing all three rounds.
  • The survey underwent significant revision, with 46 items retained, 58 revised, 2 dropped, and 8 added.
  • A validated 114-item survey was established for use in CF care centers.

Conclusions:

  • A validated survey instrument is now available for CF care centers.
  • The survey will provide insights into the benefits and barriers of key assessments.
  • Findings will support improved implementation of exercise and body composition testing for people with CF, especially in the era of CFTR modulators.
