A synthetic bone MRI protocol using spoiled gradient recalled echo (SPGRE) is effective for detecting pars defects in adolescents. This nonionizing imaging method offers good to perfect image quality for orthopedic surgeons and radiologists.
Area of Science:
- Radiology
- Orthopedics
- Medical Imaging
Background:
- Lumbar spondylolysis is a common cause of low back pain in adolescents.
- Computed tomography (CT) is often used to diagnose pars defects but involves ionizing radiation.
- There is a need for nonionizing imaging techniques to assess pars defects.
Purpose of the Study:
- To evaluate the quality of a synthetic bone MRI protocol using spoiled gradient recalled echo (SPGRE).
- To determine the utility of the SPGRE sequence in detecting lumbar pars defects in adolescent patients.
- To assess the usability of this MRI protocol for orthopedic surgeons and radiologists.
Main Methods:
- Retrospective cohort study of 30 adolescent patients with lumbar spondylolysis.
- Lumbar spine MRI at 3T including a 3D SPGRE sequence.
- Comparison with CT in 5 patients; image analysis by an orthopedic surgeon and radiologist using a Likert scale and interreader agreement calculation.
Main Results:
- Pars defects were identified in all 30 patients on SPGRE MRI.
- Excellent image quality was achieved (average score 4.53 ± 0.60).
- Substantial interrater agreement (Gwet AC2 = 0.79) was observed between the orthopedic surgeon and radiologist.
Conclusions:
- The synthetic bone MRI protocol using SPGRE is a viable nonionizing alternative to CT for diagnosing pars defects.
- The SPGRE sequence provides sufficient image quality for effective assessment by clinicians.
- This MRI protocol can aid orthopedic surgeons and radiologists in managing adolescent back pain.

Computed Tomography (CT) scan:
Computed Tomography (CT) scans use X-ray technology to generate detailed images of bones, organs, and tissues. During the scan, the patient lies on a moving table...
Computed Tomography
The technique was invented in the 1970s and is based on the principle that as X-rays pass through the body, they are absorbed or reflected at different levels. In the technique, a patient lies on a motorized platform while a computerized axial tomography (CAT) scanner rotates...
