Abstract:
We present the case of a 63-year-old man with severe angina despite optimal medical therapy, prior three-vessel coronary artery bypass graft surgery, and chronic total occlusions (CTOs) in three coronary vessels. He underwent complex percutaneous coronary intervention (PCI), successfully revascularizing all three CTOs in a single session. Despite a right coronary artery perforation managed with covered stents, he recovered well and was discharged the following day. This case highlights the feasibility, strategic considerations, and risks of PCI of multiple CTOs during the same procedure.
