Background:

  • Diaphragmatic eventration (DE) presents as abnormal diaphragm elevation.
  • DE is often asymptomatic but can mimic other conditions, leading to diagnostic challenges.
  • Current literature lacks robust epidemiological data on DE prevalence.

Purpose of the Study:

  • To investigate the true prevalence of diaphragmatic eventration.
  • To determine if DE is underreported in existing medical literature.

Main Methods:

  • Retrospective review of chest X-rays (CXRs) from the MIMIC-IV database.
  • Defined DE as >2.5 cm hemidiaphragm elevation confirmed on frontal and lateral views.
  • Assessed inter-rater reliability among four reviewers, including a radiologist, for DE confirmation.

Main Results:

  • 196 out of 279 (70.3%) reviewed cases met inclusion criteria.
  • Radiographic evidence of DE was found in 46 out of 196 (23.5%) patients.
  • This prevalence significantly exceeds previously reported rates.

Conclusions:

  • Diaphragmatic eventration may be more prevalent than currently recognized.
  • Identifying DE can prevent unnecessary diagnostic workups for mimicked pathologies.
  • Recognizing DE as a common anatomical variation can decrease diagnostic uncertainty.
