Local Anesthetics: Differential Sensitivity of Nerve Fibers01:24

Local Anesthetics: Differential Sensitivity of Nerve Fibers

Local anesthetics (LAs) block the sodium channels of nerve trunks, sensory nerve endings, and neuromuscular junctions. Although LAs can block all kinds of nerves, the sensitivity of nerve fibers differs according to nerve types and structures. LAs are known to block myelinated fibers faster than unmyelinated ones. Also, they block pain or sensory neurons at low concentrations without affecting the motor neurons involved in muscle contractions. This helps relieve labor pain without affecting the...

Summary
This summary is machine-generated.

Patellar tendon (PT) stiffness varies across its length, being higher proximally and distally than in the middle. Tendon stiffness measurements using tendotonometry are highly reliable across all tested PT locations.

Area of Science:

  • Biomechanics
  • Sports Medicine
  • Musculoskeletal Research

Background:

  • Patellar tendinopathy involves pain at the proximal patellar tendon (PT).
  • Understanding PT stiffness variations and measurement reliability is crucial for injury assessment.
  • Limited data exists on localized PT stiffness and measurement consistency.

Purpose of the Study:

  • To investigate differences in PT stiffness at various locations.
  • To assess the reliability of measuring PT stiffness at these locations.
  • To compare PT stiffness between male and female recreational athletes.

Main Methods:

  • Tendotonometry was used to measure PT stiffness.
  • Measurements were taken at nine locations (proximal, horizontal-midline, distal; medial, vertical-midline, lateral) in 90-degree knee flexion.
  • Triplicate measurements were performed on the dominant leg of 48 recreational athletes (24 male, 24 female).

Main Results:

  • PT stiffness was significantly higher in the proximal and distal regions compared to the horizontal-midline.
  • Stiffness was lower in the medial and lateral regions compared to the vertical-midline.
  • Intraclass correlation coefficients exceeded 0.9 and coefficients of variation were below 3%, indicating excellent reliability across all locations.

Conclusions:

  • Patellar tendon stiffness exhibits distinct regional variations.
  • Tendotonometry provides a reliable method for assessing localized PT stiffness.
  • These findings can inform diagnosis and treatment strategies for patellar tendinopathy.
