  • Musculoskeletal ultrasound (MSUS) offers critical data on rheumatoid arthritis (RA) disease activity and structural damage.
  • Integrating MSUS with clinical evaluation can enhance treatment decision-making for RA patients.

Purpose of the Study:

  • To assess how MSUS influences treatment choices in RA patients on conventional synthetic disease-modifying antirheumatic drugs (csDMARDs).
  • To evaluate the agreement between MSUS findings and the Disease Activity Score 28 (DAS28) in RA patients.

Main Methods:

  • RA patients (DAS28: 3.22-5.1) with inadequate csDMARD response underwent MSUS of 5, 28, and 78 joints.
  • Clinician and patient treatment escalation desires were assessed pre- and post-MSUS using Likert scales.
  • Treatment outcomes based on US assessment were followed for 24 months.

Main Results:

  • MSUS altered treatment escalation decisions in 66.7% of 27 patients.
  • Higher US scores correlated with treatment escalation (p < 0.05).
  • The 78-joint count (78USJC) showed the strongest alignment with treatment decisions (p = 0.009), with poor concordance between DAS28 and US scores overall, except for swollen joint count.

Conclusions:

  • MSUS integration into DAS28 assessment modified management strategies in a majority of RA patients.
  • MSUS provides valuable complementary information for guiding RA treatment adjustments.
