Synthesis and Regulation of Thyroid Hormones01:20

Synthesis and Regulation of Thyroid Hormones

7.2K
Low blood levels of the thyroid hormones — triiodothyronine (T3) and thyroxine (T4) — signal the hypothalamus to release the thyrotropin-releasing hormone (TRH). TRH then reaches the pituitary gland and stimulates the release of thyroid-stimulating hormone(TSH) into the bloodstream.
Upon reaching the thyroid gland, TSH stimulates the follicular cells' active uptake of iodide ions from the blood. The ions diffuse to the apical surface of the cells and are oxidized to iodine. The...

Functions of Thyroid Hormones

5.0K
The thyroid hormone (TH) plays a pivotal role in the intricate orchestration of physiological processes, exerting profound effects on development, metabolism, and homeostasis throughout different life stages.
TH is indispensable for the normal development and maturation of the skeletal, muscular, and nervous systems during fetal and childhood growth. It facilitates bone mineral turnover and regulates protein synthesis in developing tissues, contributing significantly to overall growth and...

Cardiomyopathy VII: Pre and Post Operative Nursing Management

285
Patients with hypertrophic cardiomyopathy (HCM) and left ventricular outflow tract (LVOT) obstruction who remain symptomatic despite optimal medical therapy may undergo a septal myectomy (Morrow procedure). This procedure involves excising a portion of the hypertrophied septum below the aortic valve using a heart-lung machine to improve blood flow through the LVOT. Effective preoperative and postoperative nursing management ensures successful patient outcomes, minimizes complications, and...

Endotracheal Tube Extubation

3.6K
Endotracheal tube extubation is a critical procedure in weaning patients from mechanical ventilation. It involves physically removing the oral or nasal endotracheal (ET) tube, marking the final step in liberating a patient from ventilatory support.
Procedure
Extubation removes the endotracheal tube (ETT) from the patient on mechanical ventilation. It requires a well-coordinated, multidisciplinary approach involving physicians, nurses, respiratory therapists, and other healthcare professionals....

Kidney Transplant II: Surgical Procedure

Preoperative ManagementThe primary goals of preoperative management in kidney transplantation are to optimize the patient’s metabolic state and prepare them for surgery through diet adjustments, necessary dialysis, and tailored medical treatment. This phase also involves comprehensive infection screening and patient education about the surgical procedure and postoperative care to improve outcomes and adherence.Medical ManagementA comprehensive evaluation is required for both the living...

Summary

Thyroidectomy in uncontrolled hyperthyroid patients showed comparable complication rates to controlled patients. This suggests selected thyroid surgeries may proceed without full biochemical control, especially in urgent cases.

Area of Science:

  • Endocrinology and Metabolic Diseases
  • Surgical Oncology
  • Thyroid Surgery

Background:

  • Current guidelines recommend achieving biochemical euthyroidism before thyroidectomy to mitigate thyroid storm risk.
  • Real-world scenarios may present challenges in fully normalizing thyroid hormone levels pre-surgery.
  • This study investigates perioperative outcomes in hyperthyroid patients undergoing thyroidectomy with varying degrees of biochemical control.

Purpose of the Study:

  • To compare perioperative outcomes between biochemically controlled and uncontrolled hyperthyroid patients undergoing thyroidectomy.
  • To evaluate the safety and feasibility of thyroidectomy in patients not fully normalized preoperatively.

Main Methods:

  • Retrospective analysis of 110 patients who underwent thyroidectomy for hyperthyroidism (September 2020-2024).
  • Patients were categorized as 'controlled' (normal fT3/fT4) or 'uncontrolled' (elevated fT3/fT4) preoperatively.
  • Demographic, perioperative, and postoperative data were collected and compared between groups.

Main Results:

  • 18% of patients were biochemically uncontrolled pre-thyroidectomy.
  • Uncontrolled patients were younger and more likely to have Graves' disease.
  • Operative times and complication rates (hypocalcemia, nerve palsy, hematoma) did not significantly differ between controlled and uncontrolled groups.

Conclusions:

  • Thyroidectomy can be safely performed in selected uncontrolled hyperthyroid patients without increased complications or thyroid storm.
  • These findings support considering surgery in urgent situations or high-volume centers even without complete biochemical control.
  • Biochemical normalization remains the standard; results require cautious interpretation due to study limitations (small sample size, single center).
