Summary

Lateral canthotomy with inferior cantholysis (LCIC) significantly reduces intra-ocular pressure (IOP) during simulated retrobulbar hemorrhage. This emergency procedure is effective in managing acute orbital compartment syndrome.

Background:

  • Regional anesthesia for ophthalmic surgery carries risks of severe complications like retrobulbar hemorrhage.
  • Prompt management of these complications, such as acute orbital compartment syndrome, is critical to prevent vision loss.
  • Lateral canthotomy with inferior cantholysis (LCIC) is a vital emergency procedure for managing these complications.

Purpose of the Study:

  • To validate a porcine head model for simulating intra-orbital hemorrhage.
  • To investigate the efficacy of LCIC in reducing intra-ocular pressure (IOP) during simulated hemorrhage.
  • To assess the pig-head model's utility as a training tool for LCIC.

Main Methods:

  • A randomized comparative study using 13 porcine heads.
  • Simulated retrobulbar hemorrhage induced by injecting hydroxyethyl starch.
  • IOP measurements taken at baseline, post-injection, and at immediate and 10-minute intervals post-LCIC or control observation.

  • The porcine head model demonstrates moderate to high fidelity for training LCIC procedures.
