Fixation and Sectioning

Two basic types of preparation are used to visualize specimens with a light microscope: wet mounts and fixed specimens.
The simplest type of preparation is the wet mount, in which the specimen is placed in a drop of liquid on the slide. A liquid specimen can be directly deposited on the slide using a dropper. Background:

  • Pelvic organ prolapse (POP) is a prevalent condition affecting up to 40% of women, increasing with age.
  • Colpocleisis is a native-tissue repair option for elderly women with advanced POP not seeking vaginal intercourse.

Purpose of the Study:

  • To evaluate the recurrence and reoperation rates of a standardized purse-string suture technique for colpocleisis.
  • To assess perioperative complications and impact on bladder function.

Main Methods:

  • Retrospective quality control study of 88 patients undergoing Total Colpocleisis (TC) or Le-Fort Colpocleisis (LFC) between 2014-2023.
  • Standardized purse-string suture technique used for both TC and LFC procedures.
  • Primary outcome: POP recurrence (POP-Q stage ≥2); Secondary outcomes: reoperation, complications, bladder disorders, incontinence.

Main Results:

  • Overall recurrence rate was 16%, with 7% in patients without prior surgery.
  • Reoperation rate was 9.2%.
  • Significant improvement in objective POP (p < 0.001) and bladder voiding dysfunction (p < 0.05). Perioperative complications occurred in 14.8% (mainly UTIs).

Conclusions:

  • Standardized purse-string colpocleisis is an effective surgical treatment for advanced POP.
  • Recurrence and reoperation rates are comparable to other techniques.
  • The standardized procedure is easy to learn and implement.
