The idiopathic inflammatory myopathies (IIM) are a heterogeneous group of systemic autoimmune rheumatic diseases that are characterized by autoimmune myositis. Over the past 50 years, a significant body of research has focused on autoantibody expression in IIM. When compared to other connective tissue diseases, the clinicoserologic phenotypes associated with these autoantibodies are relatively homogenous. Autoantibodies can also be used to predict disease manifestations, estimate cancer risk, and prognosticate disease course and treatment response in IIM. In the following review, we will discuss the autoantibodies that are found in patients with IIM, as well as their role in predicting disease course.
