We report a case of a 50-year-old male presenting with class III angina on exertion, where an electrocardiogram-gated coronary computed tomography angiography (CCTA) demonstrated the unique coexistence of a type IV dual left anterior descending artery and a split right coronary artery. This case underscores the importance of CCTA in accurately delineating rare coronary artery anatomical variants to avoid both a misdiagnosis and the risk of potential catastrophic outcomes during interventional or surgical procedures.
