Ewing sarcoma is a malignancy belonging to the group of small round blue cell neoplasms. Extraskeletal Ewing sarcoma is a very rare malignancy, and very few reports on this topic exist. Bone tumors are most commonly described in the 2nd and 3rd decades of life, and occur predominantly in males. The age of onset for extraskeletal Ewing sarcoma is reportedly greater. We present the case of a 3-year-old girl diagnosed with primary pulmonary Ewing sarcoma with metastasis to the contralateral lung. To our knowledge, there are no reported cases at this age.
