Anterosuperior rotator cuff tears involve the supraspinatus and subscapularis tendons, but repair should be approached as a single entity using the comma tissue as a guide for both tear recognition and reduction. These tears can be challenging to accurately identify and repair, particularly if they are chronic and/or retracted. The authors present their technique for identification and repair of retracted anterosuperior rotator cuff tears augmented using the transposed biceps tendon.
