High-Sensitivity vs Conventional Cardiac Troponin T in Non-ACS Hospitalized Patients: Impact on Resource Use and

Background:
High-sensitivity cardiac troponin T (hs-cTnT) enables earlier and more sensitive detection of myocardial injury compared with conventional troponin assays. Although extensively studied in acute coronary syndrome (ACS), its role in non-ACS hospitalized populations remains less clear.
Project Rationale:
In patients admitted without ACS, hs-cTnT may improve diagnostic efficiency and reduce unnecessary testing, but it may also increase detection of ambiguous troponin elevations, leading to resource overuse. Assessing its real-world impact is therefore essential.
Project Summary:
We conducted a retrospective study of 520 hospitalized patients at St Mary's General Hospital (January-May 2023). Propensity score-matched cohorts (260 hs-cTnT vs 260 cTnT) were compared; hs-cTnT use was associated with shorter length of stay (7.3 vs 10.9 days, P < 0.01), reduced echocardiography (76.9% vs 88.8%, P = 0.005), and fewer electrophysiology procedures (2.3% vs 6.5%, P = 0.033), with no group differences in mortality.

Findings indicated that hs-cTnT streamlines inpatient care, reduces resource use, and maintains safety, supporting its broader adoption beyond ACS.
