Moyamoya Disease in an Eight-Year-Old Female Child-A Case Report

An eight-year-old girl presented with a first episode of focal seizure and transient loss of consciousness. Brain MRI revealed a large subacute infarct in the left fronto-parieto-temporal region, and MR angiography showed bilateral supraclinoid internal carotid and proximal posterior cerebral artery narrowing with extensive basal collateral formation, consistent with Moyamoya disease. The absence of aneurysm or arteriovenous malformation excluded other vascular anomalies. She was managed with hypertonic saline, intravenous antibiotics, and antiplatelet therapy, followed by stabilization and referral for surgical revascularization. This case underscores the importance of maintaining a high index of suspicion for Moyamoya disease in children presenting with new-onset seizures or stroke-like episodes. Early neuroimaging and multidisciplinary management are essential to prevent further ischemic injury and optimize neurological outcomes.
