A mHealth app for sarcoidosis-associated fatigue: a feasibility randomized control trial

Background:
Sarcoidosis associated fatigue (SAF) is the most reported symptom by sarcoidosis patients and leads to impaired quality of life (QOL). It is strongly associated with stress and negative mood states, and accessible and effective treatments are lacking. mHealth has emerged as a viable platform for QOL self-management interventions. We sought to evaluate the feasibility and preliminary efficacy of a mHealth App, the Sarcoidosis Patient Assessment and Resource Companion (SPARC) App, to improve SAF.
Methods:
Patients with a fatigue assessment scale (FAS) score ≥22 were randomized to either standard of care or the SPARC App which included modules containing educational videos and breathing awareness meditation (BAM). After completing the education modules, patients were instructed to use the BAM module daily for three months. Primary outcomes of usability and feasibility were assessed at three and six months.
Results:
Baseline fatigue and stress scores were similarly elevated in the 49 randomized patients. While usability scores were generally favorable (uMARS functionality 3.7, engagement 3.1), only 40 % of patients in the SPARC group met the pre-defined criteria for adherence. We explored potential sociodemographic influences and key informant interviews provided insight into facilitators/barriers to app use. Fatigue, stress, and QOL scores improved in the SPARC group compared to controls, with a dose-response relationship observed in adherent versus nonadherent patients.
Conclusions:
Our data suggest mHealth apps may be effective in reducing fatigue and stress and improving QOL in people with sarcoidosis, but deployment of mHealth tools still faces barriers.
