Men diagnosed with abdominal aortic aneurysm (AAA) have higher cancer rates and worse survival, especially active smokers. This highlights the need for targeted cancer screening in AAA patients.
Area of Science:
- Cardiovascular Medicine
- Oncology
- Public Health
Background:
- Population-based screening for abdominal aortic aneurysm (AAA) in 65-year-old men reduces mortality.
- Men with AAA have increased mortality from cardiovascular disease and cancer compared to the general population.
Purpose of the Study:
- To determine the prevalence of cancer in men diagnosed with AAA.
- To assess aneurysm progression in AAA patients with cancer.
- To compare survival rates in men with AAA, with and without cancer.
Main Methods:
- A population-based longitudinal study of men diagnosed with AAA between 2010 and 2024 in Stockholm.
- Data collected from a computerized regional AAA screening database and hospital registries.
- Median follow-up duration of 7.2 years.
Main Results:
- Of 1,151 men diagnosed with AAA, 24.6% (283) were diagnosed with cancer.
- Aneurysm diameter was smallest in cancer-free patients and largest in lung cancer patients.
- Five-year survival was significantly lower for patients with cancer (79.2%) compared to those without (94.7%), particularly active smokers.
Conclusions:
- Cancer and its treatments may affect the natural history of AAA.
- Elevated mortality in AAA patients with cancer, especially active smokers, is concerning.
- Consideration for intensified cancer screening and potential modification of AAA treatment timing in this subgroup is warranted.
