A Novel Three-Step Collagen-Augmented Periorbital Comprehensive Treatment: Raise, Enhancement, and Depigmentation

Background:
The periorbital area is one of the first to show signs of aging, including pigmentation, wrinkles, puffiness, and fine lines, affecting both appearance and skin health. This study presents a novel three-step collagen-augmented treatment for periorbital rejuvenation.
Methods:
Sixty-five Chinese Han female patients and three Chinese Han male patients (25-55 years old) with periorbital pigmentation, tear trough deformity, or fine lines were treated with a three-step collagen injection method: raise (lifting and firming periorbital and midface skin), enhancement (reducing tear trough hollowness and smoothing transitions between the lower eyelid and cheek), and depigmentation (reducing pigmentation for improved skin tone). Two independent physicians assessed periorbital aging at baseline, 3, and 6 months post-treatment using Barton grade and Tear Trough Rating Scale (TTRS). Primary outcomes (pigmentation and capillary density changes) were analyzed via 3D reconstruction. Patient satisfaction was measured by FACE-Q scores, and adverse events were recorded.
Results:
All patients achieved satisfactory esthetic outcomes with no significant adverse events. Minor side effects, such as bruising, were observed. At 3 and 6 months post-treatment, significant improvements were noted in dark circles, tear troughs, and fine lines. Pigmentation indices significantly decreased from baseline values of 22.88 ± 5.04-19.08 ± 4.75 at 3 months and to 19.20 ± 4.64 at 6 months (P < 0.025). Capillary density also decreased, indicating thickening of the skin barrier and improved skin health. FACE-Q scores reflected high levels of patient satisfaction.
Conclusions:
This novel collagen treatment effectively improved periorbital aging with high patient satisfaction and safety.
