Combined Vacuum-assisted Closure Therapy and CACIPLIQ20 for Chronic Wound Management

Abstract:
Chronic wounds, defined as wounds that fail to heal within 12 weeks, pose significant clinical challenges. Management follows the tissue, infection, moisture, and edge principle, which emphasizes tissue debridement, infection control, moisture balance, and wound-edge optimization. This case series examined the use of vacuum-assisted closure (VAC) therapy combined with CACIPLIQ20 (OTR3, Paris, France), a regenerating tissue analog, in 4 patients with chronic or complex wounds. The first case involved a 13-year-old boy with delayed healing and hardware exposure after scoliosis correction surgery, successfully treated with VAC therapy and CACIPLIQ20. The second case described a 77-year-old man with a recurrent malignant peripheral nerve sheath tumor requiring surgical excision and a skin graft, which healed with VAC therapy and CACIPLIQ20. The third case featured a 14-year-old girl who developed vertebral osteomyelitis and a wound infection after kyphosis correction surgery, managed with debridement, VAC therapy, and CACIPLIQ20. The fourth case involved a 55-year-old man with diabetes and a necrotic foot wound requiring below-knee amputation, followed by VAC therapy and CACIPLIQ20, resulting in successful closure. CACIPLIQ20 restores heparan sulfate function, enhances extracellular matrix integrity, and accelerates wound healing. This case series supported the use of VAC therapy and CACIPLIQ20 as a promising approach to chronic wound management.
