Acute decompensated heart failure (ADHF) is a major cause of hospitalizations, driving extensive research on the optimization of pharmacologic management aimed at achieving effective decongestion and improving short- and long-term outcomes. Management of ADHF has predominantly relied on loop diuretics, but this approach often leads to insufficient decongestion, resulting in recurrent hospitalizations. Recent efforts have focused on tailoring decongestive strategies to individual patients. This review presents an overview of the current therapeutic landscape by analyzing recent developments and clinical trials and subsequently offering new insights into treatment strategies. The proposed tailored approach, which prioritizes patient-specific factors is aimed at optimizing treatment efficacy and providing practical guidance for the management of ADHF.
