Bioengineered Exosomal Hair Growth Factor Complex in Upregulating HFDPCs and Downregulating Il-6 and IL-1β in Hair

Background:
This study evaluated the safety and therapeutic efficacy of a novel hair growth factor complex that combines bioengineered exosomes with the biomimetic polypeptides in QR678 Nexo to treat androgenetic alopecia. Its effectiveness was rigorously evaluated through both in vitro and in vivo experimental paradigms.
Methods:
Human follicle dermal papilla cells (HFDPCs) were cultured in vitro and treated with the novel formulation. Cell viability and proliferative responses were quantified using MTT assays. Anti-inflammatory efficacy was assessed by measuring mRNA expression levels of interleukin (IL)-6 and IL-1β. Then, an open-label prospective clinical study including 85 human participants (Indian men and women between 20 and 60 years of age) was performed, with efficacy metrics including hair pull tests, photographic evaluations, videomicroscopic assessments, and patient-reported outcomes across 8 treatment sessions. Safety profiles were monitored through physical examinations and participant-reported adverse events.
Results:
The in vitro findings showed a statistically significant enhancement in HFDPC proliferation (172.4% relative to control) and downregulation of IL-6 and IL-1β mRNA levels. In vivo, the hair pull test revealed a reduction in hair shedding, from an average of 7.8 to 1.4 hairs. Videomicroscopic analyses indicated elevations in terminal hair counts and shaft diameters, with sustained enhancements in hair density 2 months after treatment.
Conclusions:
The bioengineered exosomal hair growth factor complex is a promising minimally invasive intervention for managing hair loss. It demonstrates enhanced therapeutic efficacy while minimizing adverse effects, marking a significant advancement in regenerative dermatology.
Clinical Relevance Statement:
In patients with androgenetic alopecia, treatment with a bioengineered exosomal hair growth factor complex was associated with reduced hair shedding and improved terminal hair count, shaft diameter, and photographic assessment scores. This approach may offer a promising adjunctive regenerative option, although controlled long-term studies are needed.
