Recent interest in gamma sensory stimulation as a therapeutic tool for Alzheimer's disease has accelerated its clinical translation, often at the expense of critical experimental groundwork. The study by Ke, Lo, and Tseng demonstrates that attentional state strongly modulates 40 Hz entrainment, challenging the assumption of uniform efficacy. This commentary highlights these and related findings, underscoring the urgent need for systematic research into the cognitive and neurophysiological variables that shape entrainment. Bridging this gap is essential to ensure that gamma sensory stimulation progresses from a promising concept to a truly effective intervention with the potential for tailored clinical application.
