Abstract:
Objective: To understand the basic vaccination status of human papillomavirus (HPV) vaccine in women aged 9-45 years in Shanghai from 2017 to 2024, and provide data support for the development and improvement of HPV immunization strategies. Methods: Data of HPV vaccine doses reported monthly in Shanghai from 2017 to 2024 were collected through the Shanghai Vaccine Management and Vaccination Service Information System. The proportions of HPV vaccine doses, as well as the estimated cumulative first-dose and full-course vaccination rates, were calculated for women of eligible age in different years, age groups, and areas in Shanghai. Results: From 2017 to 2024, a total of 5.819 million doses of the HPV vaccine were administered to women in Shanghai, including 3.585 million doses of the nine-valent HPV vaccine (61.61%). The doses of the vaccination increased year by year, although a decline was observed in 2024. The highest vaccination rate was in women aged 20-24 years, accounting for 22.05% of all the doses. Women aged 9-14, 15-19, 20-24, and 40-45 years tended to prefer the nine-valent HPV vaccine, while women aged 25-39 years received more quadrivalent HPV vaccine. The proportions of people in all age groups choosing the nine-valent HPV vaccine increased rapidly in 2023 and 2024. In Shanghai, the estimated first dose coverage of HPV vaccine in eligible women was 36.96%, with a completion rate of the full vaccination series reaching 93.58%. Notably, the cohort of women aged 9-14 years exhibited the lowest rates for both the first dose and series completion in all the age groups, with rates of 14.10% and 85.77%, respectively. Conversely, the highest estimated first dose coverage (88.64%) was observed in women aged 20-24 years, whereas the highest series completion rate (94.15%) was found in age group 25-29 years. Furthermore, urban area demonstrated higher vaccination rates, with first dose and series completion rates of 57.39% and 94.11%, respectively. Conclusions: HPV vaccine coverage in women aged 9-45 years in Shanghai was higher than the national average, but still significantly lower than the average in high-income countries, especially in women under 20 years old.
