Summary

Orthodontic mesialization of the third molar (M3) effectively replaced a missing mandibular second molar (M2). This minimally invasive technique restored oral function and offers a viable alternative to conventional treatments.

Background:

  • Orthodontic mesialization of the third molar (M3) presents a promising, noninvasive alternative.

Purpose of the Study:

  • To evaluate the efficacy of orthodontic mesialization of the mandibular third molar (M3) to replace a missing mandibular second molar (M2).
  • To assess the outcomes of using a segmental arch with temporary anchorage devices for M3 mesialization.

Main Methods:

  • A patient with severe caries in the mandibular right M2 underwent orthodontic examination and diagnosis.
  • The treatment involved mesialization of the mandibular right M3 using a segmental arch and temporary anchorage device over 20 months.
  • Post-mesialization, 10 months of occlusal adjustment and 36 months of natural retention were performed.

Main Results:

  • Successful restoration of occlusal function on the right side.
  • Favorable root parallelism of the mesialized M3 was achieved.
  • Preservation of the occlusal plane and facial profile was maintained.

Conclusions:

  • Segmental arch combined with temporary anchorage devices effectively mesializes vertically positioned, two-rooted mandibular M3s.
  • This orthodontic approach provides a minimally invasive, low-risk solution for replacing prematurely lost M2s in young adults.
