Background:
Sinonasal small cell neuroendocrine carcinoma (SCNEC) is a rare and aggressive tumor lacking standardized treatment. While concurrent chemoradiotherapy or surgery with adjuvant therapy is commonly employed, their feasibility may be limited in patients with prior irradiation or significant comorbidities.
Case Report:
We report a case of cT4aN0M0 sinonasal SCNEC in a patient with a history of irradiated nasopharyngeal carcinoma, ipsilateral carotid stenosis, and contralateral visual loss. Due to contraindication to radiotherapy, a sequential approach consisting of induction cisplatin-etoposide chemotherapy was adopted. The tumor showed marked regression (ycT3), enabling curative endoscopic resection with negative margins and preservation of critical structures. Oral etoposide was administered postoperatively, and the patient remained disease-free at 12 months without complications.
Conclusion:
This case highlights the feasibility of a radiation-free sequential strategy integrating induction chemotherapy, surgery, and adjuvant oral chemotherapy in selected patients with locally advanced sinonasal SCNEC who are ineligible for standard regimens.
