Purpose of the Study:

  • To assess the validity of International Classification of Diseases, 10th Revision (ICD-10) codes in identifying functioning and nonfunctioning pituitary neuroendocrine tumor (PitNET) subtypes.
  • To evaluate the association between the implementation of the WHO 2017 and 2022 classification updates and the accuracy of ICD-10 coding for PitNETs.
  • To compare the performance of ICD-10 codes under pre-2017 versus updated WHO classification criteria.

  • Analysis of 1,096 surgically treated PitNET patients from January 2020 to April 2024.

  • Implementation of the 2017/2022 WHO criteria led to a significant decrease in sensitivity and NPV for nonfunctioning PitNETs and decreased specificity and PPV for corticotroph PitNETs.
  • Coding discrepancies were attributed to new pathological terminology and misapplication of endocrine codes, particularly for pituitary stalk compression effects.

Conclusions:

  • Updated WHO classifications, while enhancing pathological detail, negatively impact the accuracy of clinical ICD coding for surgically managed PitNETs.
  • Future classification frameworks must bridge the gap between pathological nomenclature and clinical endocrine function within the ICD system.
  • Multidisciplinary collaboration and standardized coding protocols are imperative for improving the accuracy of PitNET coding.
