Background:

Emerging methods for effective patient engagement (PE) are continually being developed.
Existing studies on PE barriers lack specificity for individuals with lived experiences in pregnancy or conception.
Understanding these unique challenges is crucial for advancing perinatal health research.

Purpose of the Study:

To identify and describe the methods used for engaging patient partners in perinatal health research.
To synthesize current practices and challenges in patient engagement within this specific population.
To inform future strategies for more inclusive and effective perinatal research.

Main Methods:

Comprehensive literature search across Ovid, Web of Science, CINAHL, and grey literature from inception to October 2025.
Inclusion criteria focused on studies detailing PE processes in perinatal health research, defining patient partners by lived experience.
Analysis of 164 included studies, with a significant majority (78.7%) published within the last five years.

Main Results:

Compensation for patient partners was reported in two-thirds of studies (n=111).
Key barriers included language, diverse population recruitment, and technology access issues; loss-to-follow-up was a major limitation.
Facilitators comprised offering diverse engagement formats, avoiding jargon, and training research teams in PE strategies.

Conclusions:

There is a critical need for enhanced reporting standards for patient engagement in perinatal health research.
Further investigation is required to develop and validate methods for measuring the impact of PE on research outcomes.
Improving PE measurement will enhance study design, execution, and findings in the field.
