Summary

An impenetrable proximal cap complicates chronic total occlusion (CTO) percutaneous coronary intervention (PCI). A novel retrograde balloon-assisted subintimal entry (BASE) technique successfully treated a CTO case with both proximal and distal cap impenetrability.
