Abstract:
The main missions of coordination support mechanisms are to provide guidances to professionals, to coordinate healthcare pathways, and to coordinate activities within the region. Coordination support mechanisms are essential for enabling people to remain at home by facilitating access to care and supporting professionals, which allows patients to live under better conditions while providing valuable support to caregivers.
