Influenza A viruses (IAVs) are contagious respiratory pathogens that cause annual epidemics and occasional pandemics. Here, we describe a protocol to track viral infections in vivo using a novel recombinant luciferase and fluorescence-expressing bi-reporter IAV (BIRFLU). This approach provides researchers with an excellent tool to study IAV in...

Summary

Secondary infections of perirenal hematomas are rare but serious. This case highlights the importance of considering delayed-onset infection in patients with renal trauma, especially those with comorbidities, for timely intervention.

Area of Science:

  • Nephrology
  • Infectious Diseases
  • Radiology

Background:

  • Perirenal hematomas, often resulting from renal trauma, are common.
  • Secondary infections of these hematomas are infrequent and frequently misdiagnosed.
  • Prompt recognition and management are crucial for patient outcomes.

Purpose of the Study:

  • To present a case of delayed-onset Klebsiella pneumoniae infection in a perirenal hematoma.
  • To emphasize the diagnostic challenges and management strategies for infected perirenal hematomas.
  • To highlight the importance of considering infection in patients with a history of renal trauma and relevant risk factors.

Main Methods:

  • A case presentation of an 82-year-old female with diabetes and liver cirrhosis.
  • Initial symptoms included fever, malaise, and back pain.
  • Diagnostic imaging utilized contrast-enhanced computed tomography (CT) to identify an 8-cm gas-containing perirenal hematoma.

Main Results:

  • Blood cultures identified Klebsiella pneumoniae as the causative pathogen.
  • Initial treatment with intravenous antibiotics was insufficient.
  • CT-guided percutaneous drainage was ultimately required for successful clinical and radiological resolution.

Conclusions:

  • Delayed-onset infection in traumatic perirenal hematomas should be considered in high-risk individuals.
  • Early and definitive source control is essential for managing infected perirenal hematomas.
  • This case underscores the need for vigilance in diagnosing and treating uncommon complications of renal trauma.
