This paper reports a case of oculomotor nerve palsy treated with acupuncture based on the Linggui Bafa acupoint-opening method. The patient presented with right upper eyelid ptosis, inability to open the eye voluntarily, blurred vision, and diplopia. According to syndrome differentiation, the condition was diagnosed as spleen deficiency and liver constraint. The acupoint selected included: "Linggui Bafa" acupoints (bilateral Waiguan [TE5], Zulinqi [GB41], Gongsun [SP4], Neiguan [PC6], Zhaohai [KI6], Lieque [LU7], Shenmai [BL62], Houxi [SI3], supplemented with local acupoints (Fengchi [GB20], Yangbai [GB14], Cuanzhu [BL2], etc. on the affected side) and distal points (bilateral Hegu [LI4], Zusanli [ST36], Guangming [GB37], etc.). Treatment was administered once daily. After the first session, the patient showed marked improvement in ptosis and diplopia. Full recovery was achieved after five sessions. No recurrence was observed during a six-month follow-up.
