Transcatheter Mitral Valve-in-Valve Replacement: Overcoming an Obstacle Race

Case Summary:
A 74-year-old woman with a mitral bioprosthesis presented with acute heart failure. Mitral bioprosthesis degeneration was diagnosed, and a valve-in-valve procedure was proposed. Several difficulties arose during procedure planning, including the following questions: 1) What is the diagnosis and what treatment could be proposed? 2) What additional studies are recommended for procedural planning? 3) What measures could be taken to prevent LVOT obstruction? 4) Should a cerebral embolic protection device be used despite unfavorable aortic arch anatomy? 5) What approach could be chosen in the setting of a mobile thrombus in the left atrial appendage? 6) What are the steps to perform transcatheter mitral valve-in-valve procedure with the BATMAN technique? and 7) What is the recommended clinical follow-up for this patient? Careful planning allowed the use of different strategies to overcome these challenges, permitting the procedure to be completed successfully and without complications.
