Background:

  • The Acute Basic Palliation Concept (ABPC) is a structured model proven to enhance end-of-life (EoL) care quality in hospital settings.
  • Its applicability and impact in primary care settings remain to be fully elucidated.

Purpose of the Study:

  • To evaluate the effectiveness of the ABPC in improving symptom control and user satisfaction within a general practice context.
  • To assess the feasibility and acceptance of the ABPC among primary care providers.

Main Methods:

  • A cohort study involving 29 general practitioners (GPs) across nine practices and regional out-of-hours GP services in Northern Denmark.
  • GPs received training in an adapted ABPC, applying it to patient cases and comparing outcomes with non-ABPC managed cases.
  • Data on symptom control and satisfaction were collected via questionnaires from relatives, municipal caregivers, and GPs.

Main Results:

  • Analysis included 74 EoL care trajectories, with 51% managed using the ABPC.
  • Significant improvements in symptom control were observed with ABPC use: pain (97% vs. 83%), agitation (93% vs. 56%), and discomfort (93% vs. 81%) (P<0.05).
  • High acceptance rates were reported: 90% of GPs and 100% of municipal nurses were willing to use the concept again.

Conclusions:

  • The structured ABPC model effectively facilitated adequate symptom management in general practice EoL care.
  • The ABPC demonstrated promising potential for improving EoL care delivery in primary care settings.
  • The model was widely accepted by GPs and municipal nurses, indicating strong potential for integration.
