Summary

A new Mucin Quantitative Score (MUCQ) improves chronic bronchitis (CB) diagnosis by combining mucin concentration and composition. This novel metric offers a superior assessment of muco-obstructive lung diseases compared to total mucin concentration alone.

Background:

  • Sputum total mucin concentration previously identified as an objective marker for chronic bronchitis (CB).
  • Need for improved assessment of risk, disease onset, and progression in muco-obstructive lung diseases.

Purpose of the Study:

  • Introduce and validate a novel Mucin Quantitative Score (MUCQ) for enhanced assessment of chronic bronchitis.
  • Compare the diagnostic performance of MUCQ against total mucin concentration in sputum.

Main Methods:

  • Utilized sputum samples from the SPIROMICS cohort (n=164) with clinically defined CB.
  • Calculated MUCQ score: [Total mucin]×([MUC5AC]÷[MUC5B])÷100 μg/ml.
  • Assessed net reclassification of CB diagnosis using MUCQ versus total mucin concentration thresholds.

Main Results:

  • MUCQ score significantly reclassified CB diagnoses compared to total mucin concentration alone (P=0.001).

Conclusions:

  • The MUCQ metric is superior to total mucin concentration for distinguishing patients with chronic bronchitis.
  • Further trials are warranted to evaluate MUCQ in clinical settings for CB assessment and management.
  • MUCQ holds potential for tracking therapeutic responses in CB and other muco-obstructive conditions.
