Open-door laminoplasty can lead to loss of cervical lordosis. Preserving uppermost facet joints is crucial to prevent postoperative kyphotic deformity after this spine surgery.
Area of Science:
- Spine surgery
- Orthopedic surgery
- Neurosurgery
Background:
- Degenerative cervical stenosis often requires surgical intervention.
- Open-door laminoplasty is a common surgical technique for cervical stenosis.
- Assessing postoperative cervical sagittal alignment is critical.
Purpose of the Study:
- To evaluate cervical sagittal alignment changes after open-door laminoplasty.
- To identify risk factors for postoperative kyphotic deformity.
- To explore strategies for preventing kyphotic changes.
Main Methods:
- Retrospective review of 78 patients undergoing open-door laminoplasty.
- Radiological assessment of C2-C7 Cobb angles and alignment pre- and post-surgery.
- Analysis of CT scans for lamina fractures and facet joint disturbances.
Main Results:
- Mean cervical lordosis significantly decreased post-surgery (p=0.024).
- Uppermost facet disturbance was the independent risk factor for kyphotic change (OR 4.62, p=0.039).
- C3 involvement and lamina fractures were not independent predictors in multivariate analysis.
Conclusions:
- Postoperative sagittal alignment is affected by cranial surgical complications.
- Uppermost facet disturbance is a novel predictor of lordosis loss.
- Preserving facet joints may reduce postoperative kyphotic drift.
