Purpose of the Study:

  • To investigate the effect of pharmacologic cholinergic stimulation on segmental aqueous humor outflow in human eyes.
  • To evaluate the potential of acetylcholine (Miochol-E) in enhancing AHO.

Main Methods:

  • A prospective, interventional study was conducted on brain-dead organ donors.
  • Aqueous angiography (AA) with indocyanine green (ICG) established baseline AHO, followed by intracameral administration of balanced salt solution (BSS) or Miochol-E.
  • Repeat AA with fluorescein quantified changes in segmental AHO.

Main Results:

  • Baseline AHO was highest in the nasal quadrant.
  • Miochol-E treatment demonstrated qualitative improvement in low-flow AHO regions.
  • Miochol-E significantly increased fluorescence intensity per quadrant and per eye compared to BSS.

Conclusions:

  • Cholinergic stimulation enhances segmental AHO, not circumferential outflow.
  • Focal improvement in low-flow regions indicates a segmental drug response.
  • Findings may inform novel pharmacological and surgical approaches for glaucoma.
