    Summary

A severe case of vertebral sarcoidosis led to quadriplegia in a young man. Prompt treatment with corticosteroids and surgery resulted in recovery, highlighting a rare presentation of spinal sarcoidosis.

    Area of Science:

    • Neurology
    • Orthopedics
    • Rheumatology

    Background:

    • Sarcoidosis is a multisystem inflammatory disease that can affect the spine.
    • Vertebral sarcoidosis is rare, with limited reported cases, especially severe presentations.

    Observation:

    • A young Black male with known sarcoidosis experienced quadriplegia following a fall.
    • Spinal imaging showed extensive bone changes, including lytic and sclerotic lesions and paravertebral bony bridges, mimicking ankylosing spondylitis.

    Findings:

    • The patient recovered function after surgical intervention (laminectomy and cervical fusion) and corticosteroid therapy.
    • This case represents the most severe reported instance of vertebral sarcoidosis and is the first documented with paravertebral ossification.

    Related Experiment Videos

    Implications:

    • This case expands the understanding of vertebral sarcoidosis's potential severity and radiographic manifestations.
    • Highlights the importance of considering sarcoidosis in the differential diagnosis of spinal lesions, even with unusual features.
    • Emphasizes the potential efficacy of combined surgical and medical management in severe spinal sarcoidosis.
