Background:

Endometriosis classification systems are crucial for surgical planning and resource allocation.
Current systems lack comprehensive data correlating classification stages with operative times.
Accurate prediction of surgical duration is essential for efficient operating room management.

Purpose of the Study:

To evaluate the correlation between the rASRM, #Enzian, and AAGL2021 endometriosis classification systems and the duration of laparoscopic endometriosis surgery.
To determine which classification system most accurately predicts operative time for improved surgical planning.
To assess the impact of endometriosis staging on surgical duration.

Main Methods:

Retrospective analysis of 139 patients undergoing laparoscopic endometriosis surgery.
Classification of endometriosis extent using rASRM, #Enzian, and AAGL2021 systems.
Calculation of corrected operative time and statistical correlation analysis (Kendall's tau, Spearman's, Mann-Whitney U, generalized linear models).

Main Results:

A median corrected operative time of 68.50 minutes was observed.
Significant positive correlations were found between surgery duration and stages in rASRM and AAGL2021 classifications.
#Enzian classification also showed positive correlations, with longest operative times in surgeries affecting specific compartments (C3, A3, FI, FU).

Conclusions:

All three classification systems (rASRM, #Enzian, AAGL2021) significantly correlate with operative time in endometriosis surgery.
#Enzian and AAGL2021 classifications show potential as preoperative tools, with #Enzian offering more detailed coding.
Further research with prospective, multi-center studies is recommended to develop algorithms for enhanced preoperative planning using #Enzian codings.
