Background:

Transitions in care are critical safety points in mental health.
Communication failures with community providers increase risks of relapse, suicide, and re-hospitalization.
The Auto-Fax feature at CAMH aims to improve communication but faces inconsistent adoption.

Purpose of the Study:

To identify barriers to Auto-Fax adoption within CAMH's I-CARE system.
To explore strategies for enhancing continuity of care and provider communication.
To align with FHLIP 2026's 'From Silos to Synergy' initiative.

Main Methods:

A mixed-methods review was conducted.
Physician surveys were administered in 2023.
System analysis was performed to identify process gaps.

Main Results:

Low awareness, system glitches, and process gaps were identified as key barriers.
Physician comfort with Auto-Fax was 53%, with 65% underuse.
Only 35% of confidence notes reached their intended targets, despite regulatory standards.

Conclusions:

Inconsistent Auto-Fax uptake poses a risk to patient safety and continuity of care.
System-level strategies are needed to improve adoption and communication.
Addressing barriers is crucial for strengthening collaborative mental health care.
