Retinal Reattachment Using Suprachoroidal Viscopexy, Endodrainage and Endolaser: Technical Description With Early

Purpose:
To describe a novel modification of a suprachoroidal viscopexy (SCVEXY) technique for rhegmatogenous retinal detachment (RRD) incorporating endodrainage of subretinal fluid (SRF) and endolaser without vitrectomy.
Methods:
Consecutive case series of three young, phakic patients with primary, chronic RRD who underwent SCVEXY with a suprachoroidal viscoelastic injection at the site of the retinal break(s), combined with endodrainage of SRF through the break and endolaser retinopexy without vitrectomy. Outcomes included single-surgery anatomical reattachment, additional surgical interventions, final anatomical reattachment, and final BCVA with a minimum follow-up of 11 months.
Results:
Two of the three eyes achieved single-surgery anatomical reattachment, with no intraoperative or postoperative complications. One patient required the procedure to be repeated. At the final follow-up, BCVA improved or was maintained and anatomical reattachment was achieved in all cases with no cataract progression.
Conclusions:
SCVEXY with endodrainage and endolaser without vitrectomy is a safe, lens-preserving approach for selected RRDs. This modification addresses the issue of SRF persistence when SCVEXY is used alone in chronic regulated detachments while preserving the minimally invasive nature of SCVEXY. Larger studies are warranted to validate its long-term efficacy and role relative to traditional scleral buckling and pars plana vitrectomy.
