Pilot evaluation of a competency-based training for integrated care in specialty medicine

Introduction:
The Veterans Health Administration (VHA) has expanded efforts to integrate mental and behavioral health professionals into outpatient specialty medicine clinics. However, existing training models are tailored to primary care, and specialty medicine settings have unique differences. This project aimed to adapt and evaluate VHA's Primary Care Mental Health Integration Competency Training for mental and behavioral health professionals embedded in specialty medicine clinics.
Method:
Using a Plan-Do-Study-Act framework, the Integrated Care Competency Training program was developed and refined across two cohorts of mental and behavioral health providers integrated into specialty medicine clinics in 2024. Evaluation data included standardized patient role play ratings, satisfaction surveys, and trainer debriefs.
Results:
Across two cohorts (N = 18), all participants achieved competency certification. Cohort 2 participants demonstrated higher first-time pass rates following refinements to the training. Participants reported increased confidence, clarity around their clinical role, and high satisfaction with the interactive learning methods, including live demonstrations and structured role plays.
Discussion:
Findings support the feasibility and relevance of Integrated Care Competency Training as an adapted training program for specialty medicine settings. Early partner engagement, iterative refinement, and incorporation of adult learning strategies contributed to positive outcomes. The model demonstrates potential for broader implementation and cross-program enrichment.
Ethical Considerations:
