Abstract:
Community-based health systems integrate care within local social and geographic contexts, leveraging trust and proximity. Within local communities, pharmacists and primary care clinicians collaboratively deliver accessible, patient-centered services. Evidence from the COVID-19 pandemic and pharmacist chronic disease management programs demonstrates the effectiveness of localized, relationship-based care models in improving health outcomes.
