Objective:
To investigate the biomechanical stability of a novel self-locking oblique lateral interbody fusion (SOLIF) system applied to lumbar osteoporosis using finite element analysis.
Methods:
Three male patients (aged 76 to 81 years) with lumbar osteoporosis treated between February 2023 and March 2024 were selected. Based on their L1-S1 CT data, point cloud patch models of the L1-S1 segments were reconstructed using Mimics software. Triangular patch models were processed in Geomagic Design X to create corresponding solid models. Bone cement models were constructed according to vertebral morphology, and SOLIF instrumentation was designed based on relevant anatomical parameters. The models were finally assembled in SolidWorks 2017. Three experimental finite element models were established in ANSYS:a posterior pedicle screw-rod group,a SOLIF group, and a normal control group. A vertical load of 400 N combined with a rotational moment of 10 N·m was applied to simulate physiological spinal movements, including flexion,extension,left/right lateral bending, and left/right axial rotation. The stress on the instrumentation, vertebral bodies, intervertebral discs, and facet joints during these movements was analyzed and compared.
Results:
(1)Range of motion (ROM) at adjacent segments:Both the SOLIF group and the posterior screw-rod group showed increased ROM at adjacent segments compared to the normal group. (2)Intradiscal stress at adjacent segments:The maximum intradiscal stress at adjacent segments in the SOLIF group was lower than that in the posterior screw-rod group. (3)Maximum stress on instrumentation:The SOLIF group demonstrated a lower likelihood of instrumental failure.
Conclusion:
Compared to the posterior pedicle screw-rod system, the novel SOLIF system shows significant advantages in reducing intradiscal stress at adjacent segments and lowering the maximum stress on the instrumentation itself.
