Precision medicine in rheumatology might no longer be utopian. The past few years have seen important technological innovations in multiomics, making deep cellular and molecular characterization of the synovium possible. Efforts to identify rheumatoid arthritis endotypes with different underlying pathophysiology and response to biologic drugs have yielded promising results. Clinical trials aimed at identifying histological or molecular signatures in synovial tissue biopsies to guide treatment decisions are flourishing. This article reviews the potential of biopsy-based clinical algorithms for precision medicine in rheumatoid arthritis patients.
