Bilateral simultaneous spontaneous pneumothorax is a rare occurrence that generally requires prompt intervention. We report a case of simultaneous bilateral spontaneous pneumothorax. As the patient had minimal symptoms and normal vital signs, he was conservatively managed. Follow-up imaging showed resolution of the pneumothorax with no evidence of underlying lung abnormality or abnormal pleural communications. This rare case highlights the feasibility and safety of adopting a conservative approach in the management of bilateral spontaneous pneumothorax.
